Annual health checkup in India: what every employer must know

For many Indian employers, the annual health checkup is not a perk — it is a legal requirement. Factories, construction sites, mines and food businesses all sit under laws that mandate periodic medical examinations, with specific tests, specific forms and specific doctors. This guide separates what the law requires from what good practice adds, so you can run one programme that satisfies both.

When the checkup is legally mandatory

Under the Factories Act and the newer Occupational Safety, Health and Working Conditions Code, workers in factories — especially those in hazardous processes — must undergo pre-employment and periodic medical examinations, recorded in prescribed formats like Form 7 (health register) and certified by a certifying surgeon. Construction workers fall under the BOCW Act, and food handlers under FSSAI regulations require medical fitness certificates.

State rules add their own layers: Maharashtra, Gujarat, Tamil Nadu, Karnataka and others have factory rules specifying examination frequency (usually annual, sometimes half-yearly for hazardous processes), age-based test additions, and record-keeping duties. If you operate in multiple states, the strictest applicable rule effectively becomes your standard.

This article is general guidance, not legal advice. Requirements vary by state, process and headcount, so confirm the rules that apply to each site with your factory inspectorate or a qualified adviser.

Which tests are typically mandated

A statutory periodic medical examination generally covers: general physical examination, vision testing, blood pressure, and basic blood work. Workers in hazardous processes add exposure-specific tests — audiometry for noise, spirometry (lung function) for dust and fumes, chest X-ray where indicated, and specific biological monitoring for chemical exposures like lead or benzene.

Age bands matter: workers above 40 are commonly required to have additional cardiac and metabolic screening. A compliance miss here is not a paperwork issue — in an inspection, an incomplete health register is treated the same as no examination at all.

The documentation that saves you in an inspection

Inspectors ask for three things: the health register (Form 7 or state equivalent) with every worker's examination history, the certifying surgeon's certificates of fitness, and evidence that unfit findings were acted on — a worker moved off a hazardous line, a follow-up test completed.

Digital records are accepted as long as they are complete and producible. The failure mode we see most often is not missing checkups — it is checkups done but recorded so poorly that the company cannot prove them.

Making the mandatory checkup worth more than compliance

The statutory panel is a floor, not a ceiling. Since the logistics cost of an on-site camp is fixed — doctor, phlebotomist, setup — adding diabetes, lipid and liver screening for a modest per-head increment turns a compliance exercise into genuine early detection. Employees who discover a condition at a company checkup remember who paid for it.

Pair the checkup with a short, private report explanation. Fifteen minutes with a doctor per employee, or a tele-consultation afterwards, converts a stack of lab values into an action plan — and converts your compliance spend into visible care.

Practical checklist for HR

  1. Map your sites to the applicable law (factory, construction, food, office) and state rules. 2. List workers by category — hazardous process, age 40+, food handling — since test panels differ. 3. Book certifying surgeons early; they are scarce in many districts. 4. Schedule camps to avoid peak production days and night-shift gaps. 5. Digitise the health register from day one. 6. Diarise the next due date the day the camp ends — periodic means periodic.
