The most common question HR teams ask about annual health checkups is also the hardest to answer from a lab brochure: what should it cost, and what should it include? The honest answer is that price follows the panel — and the panel should follow your workforce, not the other way round.

What the price bands look like

In India, basic annual screening — vitals, BMI, blood sugar, a core blood panel and a doctor's review — starts around ₹199–500 per employee at scale. Mid-range panels adding lipids, liver and kidney function, thyroid and urine analysis typically run ₹500–1,500. Comprehensive and executive packages with ECG, imaging and specialist consultations go higher. Onsite camps usually beat individual lab visits on price once headcount crosses roughly 50, and they cost no workday travel time.

Match the panel to the person

A single package for everyone wastes money and misses risk. Age bands matter: under-30s need a lean panel, while 40-plus employees benefit from cardiac and diabetes markers. Gender-specific additions — such as women's health panels — and job-based tests matter too: drivers need vision and cardiac screening, and plant workers may need audiometry or spirometry that the law requires for their exposure.

This article is general guidance, not legal advice. Requirements vary by state, process and headcount, so confirm the rules that apply to each site with your factory inspectorate or a qualified adviser.

The part most programs forget

A checkup that ends in a PDF folder changes nothing. The value is in what happens after: a doctor explaining the report in plain language, a teleconsultation for abnormal findings, a diet plan, and a re-test after 90 days so you can count risks reversed — not just risks found. When comparing vendors, ask what happens after the report as carefully as you compare the price.

Getting started

Run a free health risk assessment first — it shows which risks your workforce actually carries, so the checkup budget lands where it matters. Then pick packages by employee group, choose onsite camp or partner labs, and set the follow-up loop before the first sample is drawn.
