Inspectors frequently ask for two documents in hazardous-process factories: the health register and the certificate of fitness. In many states these are Form 32 and Form 33.
Form 32: the health register
The health register records each examined worker's details, the process they work in, dates of examinations, findings and whether they were found fit. It is usually kept at the factory and produced on demand.
Form 33: the certificate of fitness
The certificate of fitness is issued after a medical examination, often by or countersigned by a certifying surgeon where the rules require it, and records whether the worker is fit for the specified process.
Form numbers vary by state
Different states number and format these forms differently. Copying another state's template is a common audit failure. Always use the format your state rules prescribe.
Keeping them audit-ready
- Link each register entry to the underlying test reports
- Track due dates for periodic re-examination
- Include contract workers doing covered work
- Keep digital backups with access controls
